PHONE CRISIS COUNSELOR

 

POSITION SUMMARY:     

Works across the organization to do what's best for the patient. This clinician will work with the 24-hour Walk-In Center, Acute Treatment Unit and/or other facilities to coordinate next level proper care. This position will work multi-disciplinary teams upon arrival to effectively meet the clinical needs of the clients; This clinical and support level position is responsible for navigating the treatment needs of clients entering our behavioral health system through telephonic and walk-in behavioral health crisis services. Works as part of a Behavioral Health Crisis and Admissions Team providing 24/7 services including screening, intervention, referral/ admission through phone and face-to-face contacts. Will utilize crisis response experience to perform, analyze and document clinical findings for care coordination. Position will provide supportive clinical service, triage and general steps related hospital admission. Includes evening/overnight shifts.

 

QUALIFICATIONS:

A: EDUCATION        Minimum of a Bachelor's degree in human services or related behavioral health field.

B: EXPERIENCE     Experience working therapeutically with specified population.

            Experience using various treatment modalities appropriate for specified population, utilizing strengths-based therapeutic approaches.

            Experience working in a team environment.

C: SKILLS     Must have sufficient language skills and be able to communicate both verbally and in writing to accurately and professionally implement and document the services provided to clients.

            Must possess basic computer skills and type 25+ w.p.m. for the input and output of client information, using electronic record.

            Ability to perform concurrent documentation.

D: OTHER     Ability to lift equipment and supplies up to 15 lbs. Must possess own transportation, have a good driving record, a valid driver's license and proof of insurance.

 

RESPONSIBILITIES:

            Customer service that promotes client centered care to include the discussion and communication with family/caretakers and other community agencies regarding the needs of the client and anticipated treatment plans.

            Work in partnership with other team members in a multidisciplinary approach and build relationships with key internal/external partners and stakeholders (medical, behavioral health, law enforcement, schools, community etc.) as a way to streamline communication and enhance integrated client care and services.

            Assess and therapeutically deescalate crisis situations. Responds to all risk calls and walk-in's in clinically sound manner that reduces harm and allows for successful secondary intervention consistent with team model and state and federal regulations.

            Professionally interpret and analyze client's psychiatric history and risk presentation in order to inform next steps which may include connection to: Crisis evaluation, risk/threat assessment, safety welfare check, or referral to outpatient or external services.

            Provide preparation of documentation and information to drive a timely crisis evaluation in order to meet regulatory responsibilities and guidelines.

            Provide clerical support to the admission, crisis evaluation and walk-in process.

            Use appropriate documentation to complete daily documentation of services provided. Responsible for maintaining the three primary internal communication venues in crisis to include the electronic crisis module (EHR), communication board and log.

            Provide collaboration and structure to the internal and external medical clearance process. Performs appropriate assessment and risk processes to include vital signs, blood alcohol levels, urine (dip, toxicity, pregnancy) and security and safety checks. Perform all aspects of clinical and environmental care that optimizes client safety and reduces likelihood of adverse events.

            Responsible for providing navigation and coordination through an entire episode of care.

            Follow AllHealth Network policies and procedures and be able to collaborate with external partners and referral source(s) procedures.

            Communicate professionally and effectively with client's fund source regarding clinical recommendations and requests for authorization for levels of care that require pre-authorization. Follow the approved guidelines to ensure utilization and reimbursement for services provided to consumers.

            Coordinate ongoing services both within AllHealth Network and to community agencies as appropriate. Including outpatient mental health services, primary care services, Emergency Respite, SALO, SAHI, ATU, and Inpatient.

            Review documentation for referrals to Bridge ATU and screen for clinical appropriateness for admission. Coordinates with consumers, referrals sources, and treatment teams to coordinate admission to Bridge.

            Support coordination with AllHealth Network treatment teams to evaluate external placement options for consumers.

            Work with CRT, and AllHealth Network treatment teams to track bed availability and efficiency of admissions.

            Provides care to consumers in a manner that promotes safety, comfort, and the therapeutic environment.

            Expected to follow all AllHealth Network clinical policies and procedures as applicable to this position, including those related to confidentiality, release of information and record keeping.

            Responsible for maintaining a complete and up to date employee file, including Relias Trainings requirements and additional trainings as instructed by direct supervisor.

            Participate in AllHealth Network team meetings and supervision as instructed by direct supervisor.

            Perform other duties within the scope of the position and the experience of the employee as may be requested or required by direct supervisor.
