Medical Care Manager

 

Position Summary:

 

Works with individuals and their families as a member of an interdisciplinary treatment team that provides community based behavioral/medical health treatment. Actively participates with the multidisciplinary team to effectively meet the behavioral health and medical care coordination needs of clients through effective assessment, care management and collaboration within scope of practice under the orders of a medical provider. Responsible for vital signs and client care pre and post medical provider visit including: obtaining all needed releases of information; procuring external medical records, medication lists, lab reports; ensuring link/communication with PCP and other external providers; making referrals and coordinating care with internal and external providers; and assisting with client education about their treatment and medications. Also provides listed support services to Telemedicine Medical Provider coordinating client care via telemedicine audio/video equipment, e mail and phone.

 

Qualifications:  

 

  1. Education: B.S.W., B.A. or B.S. in social services or human services field.

 

  1. Experience: Must have a minimum of 1 year direct clinical experience in a medical/behavioral health setting.  

 

  1. Skills: Knowledge of medical terminology and understanding of medical systems; skilled in the use of computers and office equipment; excellent communication skills - both oral and written; ability to work collaboratively and communicate effectively with internal and external staff to ensure best care for AllHealth Network clients; demonstrates excellent listening skills and provides good customer service; ability to work independently as well as be a team player; ability to problem solve and find solutions.

 

  1. Other: Ability to negotiate stairs, climb a stepladder, and lift equipment and supplies up to 25 lbs. Must possess own transportation, have a good driving record, a valid driver's license, and proof of insurance.

 

Responsibilities:

 

  1. Effectively establishes rapport, communicates, and works collaboratively with AllHealth Network clients to provide education, case management and care coordination under the orders of the medical provider.
  2. Communicates effectively both verbally and in writing to accurately and professionally implement and document the services provided to clients.
  3. Performs client assessment including taking client vital signs and assessing mental status, risk factors, current symptoms, and understanding of and compliance with treatment plan.
  4. Highly organized in obtaining all information and records needed for the medical provider's appointment with the client and any follow up needed following the medical provider appointment.
  5. Demonstrates the highest level of confidentiality standards and HIPAA compliance in the handling of customer relations and information.
  6. Provides appropriate referrals to internal and external resources establishing professional relationships and communicating effectively in the interest of our clients and enhancing community relationships.
  7. Follows all AllHealth Network clinical policies and procedures including those related to confidentiality, release of information and record keeping.
  8. Demonstrates commitment to excellent customer service.
  9. Attends and participates as an active member in AllHealth Network meetings, trainings, and supervision as directed.
  10. Completes all required documentation accurately, legibly, and within established  

timelines.

  1. Performs other duties within the scope of the position and the experience of the employee as may be requested or required by the supervisor.
  2. Effectively and efficiently operates computer, telemed and other office equipment to accomplish all required duties.
